The history form was designed for new patients to speed up their wait time at the office.  The form was designed in html, and can be printed of your computer.

To print each form, simply click the printer icon (  ) in the bottom right hand corner of the screen for appropriate print format, then select "print" from your browser menu.

Instructions:

Click the link at the bottom.  Print the history form by selecting "file" then "print". Wait a few minutes for the ink to dry, then fill out the history form and bring it with you to your visit.
